I hereby consent my Child(ren) being to be registered as a player for this season with CDJL (and certify that he/she has not registered for any other club/league) to play in the Carlow & District Juveniles League for the season. Once signed I agree to abide by the rules and policies of said league, the SFAI and the FAI. I understand that it will be necessary for the Carlow & District Juveniles League to take group and individual photos/pictures of all players as well as videotaping matches for the purposes of promoting and celebrating the activities of the Carlow & District Juveniles League in local/national media and on the league’s website/social media page(s).

The details provided by you and the player on this form will be used and stored by your Club and affiliated League and may also be shared with other Clubs, Leagues, National and Divisional associations and the SFAI/FAI for purposes of your participation in all football related activity. Your data will be stored in compliance with the Data Protection Acts and will not be shared with any other body or organisation without your consent unless such sharing of information is necessary for your participation in football competitions and activities.
