Code of Conduct as set out in the rule of Klub Kildare/KDUL Academy
I am authorising the Klub Kildare/KDUL Academy personnel to bring my child to the hospital in case of emergency (having been unable to contact either parent or number supplied) and to take all medical/surgical procedures as advised by the treating doctor.
I agree that Klub Kildare/KDUL Academy may video/photograph my child. Any video or photograph will be used for the benefit of KLUB Kildare/KDUL Academy and may be used for promoting Klub Kildare/KDUL Academy.
I agree that Klub Kildare/KDUL Academy personnel may make direct contact with my child in respect of Klub Kildare/KDUL Academy activities.
I hereby consent to give my son/daughter permission to train at KGym. I know of no reason why my son/daughter should not train. I understand that my son/daughter will engage in cardio, strength and speed training. I authorise Klub Kildare/KDUL Academy personnel to take details regarding my child's weight, strength, speed and other performance information.
I consent to the retention of this information by Klub Kildare/KDUL Academy and its use to implement a customised plan to improve athletic fitness.
I confirm that my son/daughter complies with the FAI Safer Return to Play protocol.
I have answered all questions regarding COVID-19 regarding the health of my son/daughter and I confirm that I will notify the league if there are any changes to these questions
