Medical Consent In the event of illness and injury, I give permission for medical treatment to be administered where considered necessary by a nominated first aider or by a suitably qualified medical practitioner. If I cannot be contacted and my child needs emergency hospital treatment, I authorise a medical practitioner to provide emergency treatment or medication. If a members has any medical conditions/allergies or medication the club should know, about please inform one of the coaches in writing.
