YMCA HC Child Disclaimer Notice

I, the undersigned, having enrolled my child as a member of YMCA Hockey Club and being familiar with the rules and regulations relating to hockey training and matches (i.e. the rules approved by the FIH and the training formats approved by the IHA), understand that my child/children may only partake in hockey in accordance with those rules and regulations.  I also understand that their involvement in YMCA hockey is entirely at their own risk.        

I hereby release all persons having any connection with the organisation and/or conduct of YMCA hockey training and/or matches, including the YMCA, YMCA Hockey Club, coaches, assistants, landowners, other members and other participants from any liability arising out of accidents howsoever caused resulting in damage and/or personal injury to my child/children and/or their property.        

I hereby agree for YMCA Hockey Club to use my data and my children's data for legal and insurance reasons associated with the running of the club, to provide me with club services and to receive benefits associated with being a member of the club.
