Consent for parents

Consent for parents:

1. I confirm that I am the Parent/Guardian outlined on the previous page who has the authority of consent for all participants listed in registration form.

2. I hereby consent that any of the participants included in this registration form may be conveyed (by ambulance, car or other means) to a hospital or doctor for the purpose of medical attention where such is deemed necessary by coaching staff.

3. I consent and agree that photos taken of this event may be used in accordance with current GAA guidelines.

4. I declare that all information and details furnished as part of this registration are true and correct and that Ratoath GAA shall not be held liable in contract or tort for any damage/injury arising from any omission or error on my part.

Please note that without Parental/Guardian consent, players cannot participate in the Ratoath GAA Summer Camp.
