I the parent/guardian of the above player(s) hereby consent to the use of any photograph and/or video controlled by the club of the above player(s) on publications controlled by, or consented to by, the Committee. I understand these publications include the clubs website and Facebook pages, local press and other publications that the Committee deem appropriate. I understand that this consent includes, but is not limited to, the player(s) participation in any match, award ceremony or other activity arranged by or involving the club. I state that this consent is valid until revoked by me or the above player(s) at any stage in the future. I further consent to the said player(s) being identified by name and age-group only.
