TCFC Parent/Guardian Registration/Medical/Liability Waiver

REGISTRATION WAIVER

I, the parent/guardian of the named child(ren), hereby give my approval to his/her participation in any and all activities of Termonfeckin Celtic FC. I also consent to Termonfeckin Celtic FC collecting, using and disclosing the player registration information for all soccer participation registration requirements to authorised individuals within the organization and to affiliated soccer organizations as required.  I also give consent for my child to be photographed and video recorded during the program and release use of the images and videos for Termonfeckin Celtic FC promotional purposes including marketing and social media.  I acknowledge that I have read and understood the information on this form and that the above named child shall not be considered registered until such time as all required fees, deposits and waivers have been signed and received by Termonfeckin Celtic FC.

MEDICAL TREATMENT AUTHORIZATION AND LIABILITY WAIVER

I hereby give consent to have an athletic trainer, coach, team manager, emergency mediacl technician, nurse, medical treatment facility, and/or doctor of medicine or associated personnel to provide the applicant/participant with medical assistance and/or treatment and agree to be financially responsible for the cost of such assistance and/or treatment. I understand treatment for injury will be based on information at the time. I hereby authorise emergency transportation of the applicant/participant to a medical treatment facility should an individual listed above consider it to be warranted. I recognise the possibility of physical injury associated with soccer, and hereby release, discharge, and otherwise indemnify the Club and it's officers against any claim by or on behalf of the soccer player named above as a result of that player's participation in soccer programs and/or being transported to or from the same, which transportation I hereby authorise.

• For the safety of your child, please ensure that soccer boots and shin guards are worn for both training and matches
• Parents/Guardians are welcome & encouraged to attend any/all training sessions and match's
