Parent/Guardian Consent

I am the parent/guardian of the above named child(ren).

• I understand that photographs will be taken during or at sport related events and may be used in the promotion of the sport/club.
• Each child MUST wear a helmet* for hurling, gum shields and shin guards (preferably with ankle protection) are advised for both hurling and football. Each child must also have their own club shorts and socks.

I hereby consent to my child(ren) participating in activities of the Beann Eadair GAA in line with the Clubs Codes, and The Code of Ethics for Young People in Sport as issued by the Sports Council Of Ireland. I will inform the leaders of my child(ren)s’ activities and of any changes to the information above. I confirm that all details are correct and I am able to give parental consent for my child(ren) to participate in and travel to all activities.

*APPROVED C.E. HELMETS ARE COMPUSORY FOR BOTH HURLING TRAINING AND GAMES AT ALL AGE LEVELS. REFEREES AND COACHES WILL NOT ALLOW ANYBODY TO TRAIN/PLAY WITHOUT A HELMET.  (The club will provide helmets initially where available, whilst children see if they wish to take up hurling)

**ALL CHILDREN COMPETING IN MATCHES ARE REQUIRED TO PURCHASE CLUB SHORTS AND SOCKS, WHICH ARE AVAILABLE TO ORDER AT THE CLUB.  GAME JERSEYS ARE PROVIDED BY THE CLUB AT EACH MATCH.

Data Protection Act: I consent to the details that I am being asked to supply to be retained on the computer file and to being used in connection with GAA activities by the Club.

I hereby apply to Beann Eadair CLG for membership/renewal of membership and agree to abide by the rules and regulations of the Club and to its Code of Conduct.
