I hereby apply to Raheny United Football Club for Membership of the Club and for Youth Membership of Raheny United Football Club.  I subscribe to and undertake to further the aims and objectives of the Club and of the FAI, and to abide by its Rules, and I attach herewith the appropriate membership fee as determined by the Club.

If accepted, I understand that the club, its coaches, its members and assistants shall not be held liable for any accidents, injuries or loss sustained, from any cause whatsoever or however arising, or for any property mislaid or stolen in the club or its environs.  I agree to allow photographs and videos of my child to be taken occasionally in training, competition and club events and to be published or posted to the club’s website.  By entering any e-mail address on this form I agree to accept e-mail communications from the Club.
