I understand that I am required to be present when my child/ren is/are participating in club events.
In the event of illness, if I cannot be contacted, I give permission for medical treatment to be administered where considered necessary, by a nominated First Aider or by a suitably qualified medical practitioner.
I will upload a photograph and a photo of my child’s ID to Clubforce. This will be used by designated club and FAI officials for registration purposes.
I acknowledge that the use of mobile phones or recording devices in the dressing rooms is prohibited.
I give permission for my child to be photographed during Soccer events and for such photos to be used for soccer club purposes only.
I have read the FAI’s social media policy and signed the FAI’s Social Media consent form.
I am aware that I will receive information by text message or other means regarding Club activities e.g. matches, training, meetings etc…
I am aware my contact details will be maintained by the Club Secretary for as long as I am legal guardian to juvenile Club members.
I consent to my mobile telephone number being used by team managers as part of a team group for communication purposes.
