Fill in all relevant details such as name, Address & DOB.

For email please ensure you use your own email address.

Once file is updated please navigate to the Document Wallet section of your account.

Upload the file to the CONCACAF account & choose your FA.

Your child registrations will be uploaded shortly thereafter by a Country Activator
