• As a parent/Guardian of a child attending the 2022 Summer Camp I am aware that activities during the camp are physical and accidents may occur.
• I give permission for my child to receive medical treatment by a doctor or qualified person at the venue/hospital.
• I give permission to have my child brought to a hospital or doctor in case of emergency.
• Photographs may be taken for use in Dundalk FC publications such as website, match day programmes etc.
