I fully understand the injury claim scheme, and all injuries must be reported to the Club Secretary within 24 hours. It is my responsibility to provide all information to the Club Secretary by required dates. I understand the Club Secretary is not responsible for chasing the return of forms or information.
I understand the club will cover €10.00 per visit towards the cost of two physio visits, this must be approved by the Chairperson, Secretary or Treasurer prior to the visit.
