Parent/Guardian Consent

1. I understand that photographs will be taken during or at sport related events and may be used in the promotion of the sport/club, in print & social media or website.
2. Children, (from U/6 up to U/10), must be accompanied at training by an adult at all times (either a parent/guardian). It is not acceptable to leave your child at the gate and not return until after training. Somebody must be there in the event of injury or training being cancelled e.g bad weather etc.
3. Each child MUST have a helmet for hurling/camogie and a gum shield for Football. Gum shields and shin guards (preferably with ankle protection) are advised for hurling. Each child must also have their own club shorts and socks for matches.

I hereby consent to the above child(ren) participating in activities of the Maynooth GAA in line with the Clubs Codes, NGB Rules and The Code of Ethics for Young People in Sport as issued by the Sports Council Of Ireland. I will inform the leaders of my child(ren)s’ activities of any changes to the information above.  

I confirm that all details are correct and I am able to give parental consent for my child(ren) to participate in and travel to all activities.

APPROVED C.E. HELMETS FOR HURLING AND GUM SHIELDS FOR FOOTBALL ARE COMPUSORY FOR BOTH TRAINING AND GAMES AT ALL AGE LEVELS.  REFEREES AND COACHES WILL NOT ALLOW ANYBODY TO TRAIN/PLAY WITHOUT A HELMET OR GUM SHIELD. 

All applications for membership are subject to approval by the Executive Committee.
